PEST CONTROL SERVICE AGREEMENT

Electronic Communication Acknowledgement Statement. In accordance with state regulations, pest control companies have a responsibility to provide you with a record every time a pesticide product and/or a pest system is applied. This record is required to be provided to the property owner, resident or custodian of the property. This record may include post-application precautionary information. I acknowledge that the initial Qtly service agreement includes an inside and outside preventative treatment and that all consecutive services after the initial consist of ONLY an outside preventative treatment and may accrue an additional charge if an inside preventative treatment is requested. An additional charge may not be applied for a localized targeted pest Call Back service that is included within the scope of this agreement.

PEACH TOWN PEST CONTROL AGREES TO PROVIDE PEST CONTROL SERVICES AS DESCRIBED ABOVE. THIS AGREEMENT DOES NOT COVER FLEAS, SPIDERS OR WASPS. SERVICE FOR FLEAS, SPIDERS AND/OR WASPS WILL INCUR AN ADDITIONAL COST UNLESS OTHERWISE AGREED TO. THIS AGREEMENT IS EFFECTIVE FOR ONE YEAR AND WILL CONTINUE UNTIL TERMINATED BY EITHER PARTY.

THE GEORGIA STRUCTURAL PEST CONTROL ACT REQUIRES ALL PEST CONTROL COMPANIES TO MAINTAIN INSURANCE COVERAGE. INFORMATION ABOUT THIS COVERAGE IS AVAILABLE FROM THIS PEST CONTROL COMPANY.
